The prevention, management and treatment of multiple rapidly progressing carious teeth(‘rampant caries’) in fearful frail older people with limited ability to co-operate can be very difficult and challenging. Frail patients, with and without cognitive deficits even when co-operative, are less likely to attend a second dental appointment if frightened or find their first visit too difficult. In people with advanced frailty, small or early carious lesions are often ignored. Similarly, early caries in people exhibiting challenging or resistive behaviours may be considered by some dentists as too difficult, making later treatment in both cases even more problematic and more invasive.
